==================================================================

Updated 28 October 2026

**GP chronic condition management plans** (GPCCMPs) replaced GP Management Plans and Team Care Arrangements on **1 July 2025**. One item now covers preparing a plan, [965](/items/965), and one covers reviewing it, [967](/items/967), each **$160.60** from 1 July 2026. Patients with a plan prepared or reviewed in the last 18 months can be referred for up to **5 Medicare-funded allied health services** a calendar year.

The old items, 721, 723 and 732 and their telehealth equivalents, ceased on 1 July 2025. GPCCMP items attract the standard [bulk billing incentive](/guides/bulk-billing-incentives) when bulk billed, and are on the BBPIP eligible services list.

Who is eligible
---------------

- The patient has at least one medical condition that has been, or is likely to be, present for **at least 6 months**, or is terminal.
- Plans are not available to people in residential aged care.
- Patients registered in MyMedicare must get their plan from the practice where they are registered. Other patients see their usual medical practitioner. The video and phone items use this test instead of the telehealth practitioner rule.

What the plan must include
--------------------------

The plan is written, and covers:

- the patient’s condition and health care needs;
- goals agreed with the patient;
- actions the patient will take;
- treatments and services the patient needs, including referrals to allied health and other providers;
- arrangements for reviewing it.

Get the patient’s agreement, offer them a copy, keep a copy, and get consent before sharing it with other providers.

How often
---------

- **Prepare** no more than once every 12 months, and at least 3 months after the last review.
- **Review** no more than once every 3 months, unless there are exceptional circumstances.
- Plans don’t expire. To keep accessing allied health services, the patient needs a plan prepared or reviewed in the last **18 months**.

Allied health referrals
-----------------------

- Up to **5 individual allied health services** each calendar year (10 for Aboriginal and Torres Strait Islander patients), plus services by a practice nurse or Aboriginal health practitioner.
- People with type 2 diabetes can also have 1 group assessment and up to 8 group sessions.
- Unused services don’t roll over; the count resets on 1 January.
- Referrals are written, signed and dated, give the reason, and last **18 months** unless you state otherwise. They don’t need to state a number of services.

Patients with a GP Management Plan or Team Care Arrangement from before 1 July 2025 can keep using it for allied health referrals until **30 June 2027**.
