Bulk billing incentives and BBPIP explained: 2026 amounts by MM area
====================================================================

Updated 28 October 2026

When you bulk bill an unreferred service, Medicare pays a **bulk billing incentive (BBI)** on top of the item’s benefit. From 1 November 2025 every Medicare-eligible patient attracts a BBI, not only children and concession card holders. A practice that bulk bills **all** eligible GP services can also join the **Bulk Billing Practice Incentive Program (BBPIP)**, which adds 12.5% of the MBS benefit, split between the GP and the practice.

Use the [bulk billing calculator](/bulk-billing) to see the total for any item.

How much is the bulk billing incentive?
---------------------------------------

There are three BBI categories. The amount depends on the category and the **Modified Monash (MM) area of your practice location**, which is the location linked to your provider number, not where the patient lives or where a telehealth patient is. Fees are from 1 July 2026; Medicare pays 85% of each.

Two further items, [10992](/items/10992) and [75872](/items/75872), apply only when an MM1 practice provides an after-hours service out of its rooms in an MM2–7 area.

Which category applies?
-----------------------

- **Triple BBI (category 2):** Level B general attendances face to face, by phone or by video, and Level C, D and E face-to-face consultations, including the aged care, out-of-rooms and after-hours equivalents. For example, [item 23](/items/23), 36, 44 and 123.
- **MyMedicare triple BBI (category 3):** Level C, D and E video and Level C and D phone consultations for patients registered in MyMedicare with your practice. For example, [item 91801](/items/91801) and [91900](/items/91900).
- **Standard BBI (category 1):** every other unreferred service you bulk bill, such as Level A consultations, chronic condition management plans, health assessments, mental health items, antenatal attendances, minor procedures, and longer telehealth for patients not registered in MyMedicare.

Rules for claiming a BBI
------------------------

- The whole service must be bulk billed: the patient pays nothing for this item or any other item for the same service.
- One BBI per MBS item.
- Not with pathology or diagnostic imaging, and not for admitted hospital patients.
- Any medical practitioner who bulk bills an eligible service can claim a BBI, not only GPs.

The Bulk Billing Practice Incentive Program
-------------------------------------------

BBPIP pays **12.5% of the MBS benefit** for eligible bulk-billed services, **excluding the BBI itself**. Half goes to the GP and half to the practice, paid quarterly in arrears into each party’s nominated account.

> A GP bulk bills [item 23](/items/23) in an MM1 practice in BBPIP. Medicare pays the $45.05 benefit, a $22.40 triple BBI, and 12.5% of $45.05 = $5.63 through BBPIP: $73.08 in all.

To take part:

- the practice must bulk bill **every eligible service for every Medicare-eligible patient**, register as bulk billing only on the National Health Services Directory, be registered in MyMedicare, and register for BBPIP in the Organisation Register;
- each GP must be linked to the practice and bulk bill all eligible services there.

Eligible services are a published list of GP attendance-type items, including consultations, health assessments, mental health, chronic condition management and after-hours items. No procedures are on the list, so you can still charge a gap for a skin excision. Charging a gap for an eligible service can mean no BBPIP payment for the whole quarter.

Frequently asked questions
--------------------------

### How much is the bulk billing incentive in 2026?

From 1 July 2026, the standard BBI in a metropolitan (MM1) practice is item 10990, with a fee of $8.80 and a benefit of $7.50. The triple BBI for Level B and longer face-to-face GP consultations is item 75870, with a fee of $26.35 and a benefit of $22.40. Both are higher in rural and remote areas, up to $43.15 for the triple BBI in MM7.

### Is the bulk billing incentive paid at 85% or 100%?

At 85% of its schedule fee, rounded up to the nearest 5 cents. Bulk billing incentives are not in the list of services paid at 100%, even though the GP consultations they are claimed with are.

### Which patients can I claim a bulk billing incentive for?

Since 1 November 2025, every Medicare-eligible patient, as long as the service is bulk billed, unreferred and not for an admitted patient. The age and concession card limits no longer apply.

### Does the MM area depend on where the patient lives?

No. It is the Modified Monash area of the practice location linked to your provider number. That applies to telehealth too. The after-hours items 10992 and 75872 are the only ones that also look at where the patient is.

### Does BBPIP include the bulk billing incentive?

No. BBPIP is 12.5% of the MBS benefit for the eligible item only. The BBI is paid on top, and is not part of the BBPIP calculation.

### Can a BBPIP practice still charge a gap for procedures?

Yes. Services that are not on the BBPIP eligible services list, such as procedures, can be privately billed. Every eligible service must be bulk billed for every Medicare-eligible patient.
